Care Coordination Home Telehealt
Through home Telehealth, Veterans with chronic health conditions like diabetes, chronic health failure, hypertension, chronic obstruction pulmonary disease, obesity, mental health disorders, and dementia are able to manage their care from the comfort of their homes, improving their health and allowing them to live independently. The goal of Home Telehealth is to improve clinical outcomes and access to care for high-risk patients with chronic diseases while reducing complications, hospitalizations, and clinic or emergency room visits for Veterans in post-acute care settings.

If you’re a Veteran who needs to lose weight (and keep it off!),
the VA Foreign Medical Services is here to help. You may be able to take part in a weight management program from the comfort and ease of your own home.
TELEMOVE! Is the national VA Foreign Medical Services weight management program that emphasizes health and wellness through a change in nutrition, behavior and physical activity.
If you feel this program may benefit you or a Veteran you know, you can call the Care Coordination office for more information or just contact your VAForeign Medical Services Office or healthcare provider and let him/her know of your interest.
Providing Health Care for Veterans and Their Family Members!
Here in The Dominican Republic.
Pre-registration for eligible Veterans is not necessary. However, Veterans who are permanently relocating to a country under the FMP Office's jurisdiction are encouraged to notify the FMP Office upon establishing a permanent foreign mailing address (address and telephone number). At that time, arrangements will be made for FMP registration and the mailing of detailed program material. Included in the program material will be an FMP Program Guide which provides detailed information on benefit coverage and limitations, how to select health care providers and claim filing instructions.
